Default Step 2 CS Mnemonics

Salaam all, before entering the knocking on the door to enter the patient encounter during your CS exam make sure you always write out PAMHUGS FOSs. This ensures that you dont miss out on any of the mayor questions that you will need to ask in every case.


"PAMHUGS FOSS"

This standard mnemonic helps us in completing a clinical history after the chief complaint and related relevant history-taking steps have been taken care off.

* P-ast medical history
* A-llergy hx
* M-edication hx
* H-ospitalization hx
* U-rinary problems
* G-I problems (apetite/weight)
* S-leep probs
* F-amily hx
* O-b/gyn hx
* S-exual hx
* S-ocial hx - use 'SODA'


"SODA"

'SODA' is a mnemonic to enquire about a detailed social history :


* S-moking
* O-ccupation
* D-rugs (Illicit)
* A-lcohol + CAGE questionnaire
alcoholism is more then 2 per day or more then 4 in one sitting is binge drinking.

CAGE:
-cut down
-annoyed
-guilt
-eye opener drink in the morning


"LIQORAAA"

This is a standard mnemonic for eliciting a complete history for any kind of pain chief complaint:

* L-ocation (of pain)
* I-ntensity
* Q-uality
* O-nset + Progression
* R-adiation
* A-ssociated Symptoms
* A-ggravating Factors
* A-lleviating Factors

Vomiting(any discharge):
A=amount
B=blood
c=consists of
c=color
o=odor


MMSE:

time: Who is the president?
place:Where are you?
person: Who am i?


Memory 'VANISHED'

M - Medications (Indeed, as per WebMD, "Overuse of medicines may be the single biggest cause of memory loss or confusion in older adults."

V - Vascular Events (Strokes)
A - Alzheimer's, Alcohol, Arrhythmias
N - Normal Pressure Hydrocephalus
I - Increased ICT (e.g.tumor)
S - Sub-Dural Hematoma (SDH)
H - Hypothyroidism, Hypoglycemia, Head Injury
E - Emotional Problems - Depression
D - Dementias (various types), Delirium, Drug Abuse


kids: 'oncall idiot'
o=onset
n=number of times
c=cry/cough/consistent of
a=associated symptomps(diarrhea/fever/rash)
l=lethargy
l=liquids intake/urine output
i=infection/immunization
d=diet/dehydration
i=ill contacts
o= ORS
t=travel history/eating out

also ask mom about her pregancy/delivery any complications? neonatal infections?
