Default Step 2 CS acceptable abreviations

Salaam all, here is a list of acceptable patient note abbreviations. Using them will save you time which you can then use to put more in your PN (Patient Note)


Bp=blood pressure
Hr=heart rate
R=repirations
T=temperature
Yo=year old
M or g=male
F or e=female
B=black
W=white
L=left
R=right
Hx=history
h/o=history of
c/o=complaining of
nl=normal limits
wnl=within normal limits
ae=without or no
+=positive
-=negative
Abd=abdomen
Aids=aquired immune deficiency syndrome
Ap=anteroposterior
Bun=blood urea nitrogen
Cabg=coronary artery bypass grafting
Cbc=complete blood count
Ccu=cardiac care unit
Cig=cigarettes
Chf=congestive heart failure
Copd=chronic obstructive pulmonary disease
Cpr=cardiopulmonary resuscitation
Ct=computerized tomography
Cva or tia=cerebral vascular accident or transient ischemic attack
Cvp=central venous pressure
Cxr=chest x-ray
Dm=diabetes mellitus
Ecg=electrocardio gram
Ed=emergency department
Emt=emrgency medical technician
Ent=ear, nose and throat
Eom=extraocular muscles
etOH=alcohol
ext=extremities
fh=family history
gi=gastro intestinal
gu=genitourinary
heent=head,eyes,ears,nose, and thought
hiv=human immunodeficiency virus
htn=hypertention
im=intramuscular
iv=intravenous
jvd=jugular venous distention
kub=kidney, ureter and bladder
lmp=last menstrual period
lp=lumbar puncture
mi=myocardial infarction
mri=magnetic resonance imaging
mva=motor vehicle accident
neuro=neurological
niddm=non Insulin dependent diabetes mellitus
nka=no known allergies
nsr=normal sinus rhythm
pa=posterioranterior
perla=pupils equal, react to light and accommodation
po= per oral/orally
pt=prothrombin time
ptt=partial prothrombin time
rbc=red blood cells
sh=social history
u/a=urinalysis
uri=upper respiratory tract infection
wbc=white blood cells
