This year there will be single entry test for both FSc and A levels students.

Default UHS Entry Test for Year 2011
UHS Medical Entry test will consist of 220 multiple choice questions (MCQs), and each question carrying five marks. Total time allowed for the test will be 150 minutes. There will be five sections of the question paper i.e. Physics (44 MCQs), Chemistry (44 MCQs), English (22 MCQs), Biology (88 MCQs ) and Aptitude (22 MCQs).
Negative marking will continue as to restrain students from unintelligent guesswork in the test. One wrong answer will lead to deduct one mark from the total score.

I am not sure about the date of availability, You can get UHS Entry Test Form from UHS directly or by mail. Second option is that you just go to any near by government medical college in Punjab and get UHS entry test form. People in Islamabad and Rawalpindi can get them from Rawalpindi Medical College.

MR JAMAL how are you assured that UHS TEST will commence on 13th september 2011 ?
kindly give information if u have got from somewhere abt UHS test date
