There are 2 types of NY letters....1 for residency and 1 for electives....the one for electives is mandatory for doing electives in any NY university.... its like a permission from NY state education department for your electives....its fees is 30 $; SUNY downstate requires it in advance with your application whereas all other universities of NY require it after your acceptance...the fees of 30 $ must be paid through a bank draft payable at any US bank... the 2nd NY letter is generally required for exemption of the restriction that you cannot apply for residency in NY if you have done electives for more than 12 weeks in NY before your medical school....externships are not included in these 12 weeks...anyways the latter is not 100% confirm...I would confirm and update on this later.
